- Enable convenient and flexible communication between the various participants in the pilot, including the medical practices and Healthy Heart Society of BC
- Disseminate resources for use by pilot sites
- Obtain timely concerns and feedback from pilot sites
healthyheartGMV is an initiative of Healthy Heart Society of BC designed to help support GPs to provide more comprehensive cardiovascular risk assessments for their patients. It combines the resources and learnings from our award winning, community-based heart health literacy campaign, hearts@work, and applies them to primary care using a group medical visit format.
