healthyheart_GMV is an initiative of Healthy Heart Society of BC designed to help support GPs to provide more comprehensive cardiovascular risk assessments for their patients. It combines the resources and learnings from our award winning, community-based heart health literacy campaign, hearts@work, and applies them to primary care using a group medical visit format.
